My Future, My Plan is designed to motivate and guide students with disabilities and their families as they being early transition planning for life after high school. Directed at students who have an Individualized Education Program (IEP), these materials promote positive attitudes, self-advocacy, and help make the transition planning process more effective. My Future, My Plan tells the inspiring stories of three students who overcame barriers to achieve their goals in life after high school through transition planning, positive attitudes, and self-advocacy. Their stories of determination motivate other students to explore their own options, and the discussion guide helps students and families begin to think about transition planning goals. The video may be used in home, school, and community settings. These materials are currently adopted for use in transition planning programs.
